Background Check

Blossom & Branch (the "Company") intends to obtain a "consumer report" and/or an "investigative consumer report" (commonly known as a background report) about you for employment purposes, including as part of the process for your application, contract, promotion, reassignment, or retention as an employee or contracted worker.

The background reports will be obtained from a consumer reporting agency. The report may contain information concerning your character, general reputation, personal characteristics, and mode of living.

The types of information that may be obtained include, but are not limited to

• Criminal history records

• Social Security Number verification

• Motor Vehicle Records (MVR/Driving Records)

• Verification of your education or employment history

• Other background checks

You have the right, upon written request made within a reasonable time after receipt of this notice, to request a complete and accurate disclosure of the nature and scope of the investigation requested.

Additionally, under the Fair Credit Reporting Act (FCRA), you have the right to receive a copy of "A Summary of Your Rights Under the Fair Credit Reporting Act".

This document is the official disclosure required by federal law (FCRA) and does not include your written authorization. Your written authorization to proceed with the background investigation is requested in a separate document.
